Submit Your Pre-Placement Documents
Before starting placement, students in our Personal Support Worker, Early Childhood Assistant and Food Service Worker programs must submit their medical and pre-placement documents. Please complete the form below and upload clear, readable copies of every document that applies to your program.
Accepted file types: PDF, JPG, PNG (max 10 MB each). Photos are automatically converted to PDF when you upload them.

- CPR – Standard First Aid Level C (min 6 months validity)
- Mask Fit
- BLS (Basic Life Support)
- Medical Report
- Second Dose for Medical
- Blood Work for Medical
- COVID Vaccination Report
- Chest X-Ray (if applicable)
- VSS Police Check
- Yellow Card / Proof of Immunization (if applicable)
Missing or incomplete documents may delay the start of your placement.
